If the killer runs iron grasp, it counters it completely. Don’t waste a perk slot on it. Boil Over is good in exactly 3 circumstances. 1: You're in a tight area, IE those small side rooms in Gideon. 2: You have friends prepaired to block for you. 3: You need a few extra seconds for DS. Nay. Neigh. Step 1: play normally until you down somebody. Step 2: if they have Boil Over, you will see it in the bottom right as a debuff icon, as a red square by your perks. Step 3: if the wiggling part is the problem, look sideways and then walk the direction you want. Wiggling only makes you go left and right from how you’re facing, so if you look ... boring, annoying, usually used with a bunch of hook distance offerings to troll killers. Boil over = no rights. I have used Tier 3 Boil Over quite a bit and there is no way that this perk is actually increasing struggle effects by 75%. Not even close.
Boil Over is a fairly useless perk. I think it's meant to partially counter Iron Grasp, but it doesn't do that very well. If you're on PC, learn to turn 90-degrees and use the sway of the wiggle to move the direction you want to go. Don't try to fight the wiggle if you get stuck, you have to turn and ride it.

Some people are identical as a tactic, others not. Sometimes visually identical ones will do this with other perks, like boil over, decisive strike etc. it didn’t bother me that they do it because it’s a valid tactic, but so is leaving them there. Like many perks and strategies they can be annoying but there is usually a counter. Good luck!
