It appears that the inside of the eyelet of the carrier spring is worn or the post it sits on in the trigger housing is worn, maybe a bit of both. The result is a spring that will slide off the carry assy contact point after one or two discharges. The spring wedges itself between the carrier assy and the receiver wall cavity.

Pull the keeper out of the mag tube being carefull of the spring tension. swab the tube out real good, make sure the follower is slipping nice and free the length of the tube, put a little lube on it and reassemble.
