Its caused by nearby parasites and it depends on the strength of the parasite (don't know the proper term), basically there are 3 stages of fear -1. You cant use items that right click, 2. You can't place blocks, 3. You can't use your inventory. (I think that's how they work). And you can't really do anything about it except avoid the parasites.
